      If you mean what information you put on a blood bottle, the Laboratory User manual describes what is required for identifying who the blood sample belongs to e.g. if you take blood from me, you must write my full forename, surname, DOB and chart number. These details must match what is written on the request form that accompanies the blood bottle – I hope I’m answering your question correctly??
